Appointments
If you need to reschedule or cancel your appointment, please call as soon as possible so that this time may be available to other patients. The goal of this practice is to provide you and your fellow patients with the highest quality of health care in a timely manner.
Due to the number of "no-show" patients, it has become necessary to implement the following appointment/cancellation policy.
If you fail to cancel your office appointment at least 24 hours ahead or fail to show up for your scheduled appointment, you will be charged a "no-show" fee in the amount of $50.This is non-covered by your insurance company, and the fee will be your financial responsibility.
Any child under 18 years of age must be accompanied by their parent or legal guardian for their first visit. It is preferred that a parent or legal guardian accompany children coming for a follow-up visit. If this is not possible, written arrangements made in advance are necessary.
Insurance
We participate in most insurance plans and are happy to file claims to the insurance carrier you have specified. We suggest you call your insurance company prior to your visit in order to understand your coverage. If your insurance plan requires a referral, you will nee to obtain one from your primary care physician prior to your visit.
Please tell us if your insurance has changed since your last visit. You will be responsible for claims denied due to insurance changes that were not presented prior to care.
Payment for Services
Payment for deductibles, co-payments, co-insurance, and non-covered services are expected at the time of service. Account balances are your responsibility and must be paid prior to being seen. For your convenience, we accept cash, checks and major credit cards. Please let us know of any financial situation that you would like us to take into account.
Confidentiality
Your medical records are strictly private. We will release information regarding your condition or treatment only upon your written request or by Florida Law.
